to find a qualified Case Manager RN in Anchorage, Alaska, 99508!
Job Title: Case Manager RN Profession: Registered Nurse Specialty:
Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8
hours (5 days a week, 07:00 - 17:00) Experience: Minimum of 1 year
required License: Active RN license required Certifications: BLS
required, ACM certification preferred Must-Have: Must have Epic
experience. Must have Acute Care experience. Description: The Case
Manager RN will be responsible for discharge planning. Experience
in an acute care hospital setting is required. Behavioral health
experience does not apply. Epic Hardstop requirement must be met.
Microsoft Office/Teams experience is preferred. Candidates must be
willing to work every other weekend or 4 weekend days in 4 weeks.
The clinician may float within the case management services to meet
facility needs. Candidates must be prepared to work during 2 out of
the 3 major holidays: Thanksgiving, Christmas, and New Year.
References are required. Travelers must look into housing options
in the area, as options may be limited. Candidates will be
responsible for submitting required documentation. The job entails
maintaining compliance with all applicable laws, regulations, and
guidelines. All duties must be performed in accordance with
established policies and procedures. Candidates must have strong
skills in care coordination, discharge planning, and assessment. 5
