For any LTC CNA positions, candidates may submit with a license pending, but must meet the following criteria: Must have a valid CNA license in another state. This includes: Verified application submission (must provide written proof) Submission of application fee. Note Application fee is $275. Facility only accepts Hep B vaccines and will not take titers or declination If candidate does have documentation of Hep B vaccine they must be willing to be vaccinated again or seek a medical exemption. Medical exemption requires a written exemption from a medical provider. Documentation of candidate s willingness to obtain Hep B vaccination or ability to obtain medical exemption must be provided upon signing if they do not have Hep B vaccination records. REFERENCES - Minimum of 2 supervisory references covering 1 year (6 months for CNA s) of employment total within the last three years within a comparable specialty/setting that they are booked. This can include Charge RNs / Leads. For LTC CNA, they must have a reference from LTC work experience. For any LTC positions, candidate is required to have LTC experience. If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency. If a candidate is accepting facility-provided housing (note this is not available with all positions), they are not eligible for housing stipends. If facility-provided housing is not available, the candidate must confirm upon signing that they have obtained their own housing accommodations. Note that housing in this area is very limited.
