hours (07:30 - 16:00) Experience: Minimum 1 year of acute care
experience License: RN license in hand required Certifications: BLS
required, ACM certification preferred Must-Have: Must have Epic
experience. Must have acute care experience. Description: The Case
Manager RN will be responsible for discharge planning and care
coordination. This role requires working with interdisciplinary
support teams including physical therapy, social services, and
pharmacy. Candidates should be comfortable floating within the case
manager services to cover gaps. Weekend shifts are required.
Candidates must be open to meeting department needs by working
every other weekend or four weekend days in a four-week schedule.
References are required from supervisory roles covering the last
three years of employment in a comparable specialty/setting.
Experience with Epic electronic medical records is required.
Candidates must demonstrate familiarity with care coordination,
discharge planning, and needs assessments. BLS certification is
required. Candidates with ACM certification are preferred.
Knowledge of CMS guidelines and HIPAA regulations is essential.
This position does not require on-call duty. Local candidates are
not accepted; candidates must reside at least 50 miles away from
the facility. Travelers are expected to work two of the three major
holidays: Thanksgiving, Christmas, and New Year. Housing options
may be limited, and candidates are encouraged to explore their
accommodations. This role does not allow for block scheduling.
