Title: Registered Nurse (RN) – Case Manager (Discharge Planning)
Location: Anchorage, AlaskaContract Duration: 13 WeeksShift: Day
Shift (5x8s; 07:00–17:00)Position Overview We are seeking a
Registered Nurse Case Manager with strong acute care discharge
planning experience to support patient care coordination in a
fast-paced hospital setting. This role focuses on ensuring safe,
timely, and efficient patient transitions across the continuum of
care. Important: Candidates must have recent acute care hospital
experience. Behavioral health-only experience will not be
considered. Key Responsibilities - Lead discharge planning for
patients across all age groups - Perform care coordination with
interdisciplinary teams - Conduct patient assessments and develop
individualized care plans - Arrange durable medical equipment (DME)
and post-discharge services - Ensure compliance with CMS and HIPAA
regulations - Collaborate with physicians, nursing staff, and
support services - Utilize Epic EMR for documentation (required) -
Apply Milliman Care Guidelines (MCG) for care planning (preferred)
Work Environment - Acute Care Hospital (Level II Trauma, Magnet
Facility) - Daily Census: ~324 patients - Patient Ratio: ~1:50 -
Float required within case management services - Strong
interdisciplinary support including therapy, pharmacy, social
services, and more Required Qualifications - Active Alaska (AK) RN
License - Minimum 1 year acute care hospital experience (required)
- Epic EMR experience (Hardstop) - BLS certification (required) -
Experience in: - Discharge planning - Care coordination - Patient
assessment and plan of care Preferred Qualifications - ACM-RN
Certification (Accredited Case Manager) - Experience with: -
Pediatrics or Women’s Services - ICU or Stepdown units -
Familiarity with: - Microsoft Office / Teams - Milliman (MCG)
guidelines Schedule & Requirements - Monday–Friday business hours
(primary schedule) - Weekend Requirement: Every other weekend or as
needed - No on-call requirement - Must be flexible with shift times
within day schedule Patient Population - Neonates - Infants -
Pediatrics - Adolescents - Adults - Geriatrics Orientation - 24
hours Additional Expectations - Must be comfortable in a
high-volume environment - Must demonstrate acute care experience
clearly in work history - Ability to float within department as
needed - Compliance with all facility and regulatory standards
