Care experience Description: The Case Manager RN will be
responsible for discharge planning. Candidates must possess an
active RN license. BLS certification is required. Candidates are
required to have a minimum of 2 years of experience. First-time
travelers are accepted. Weekends are required, every other weekend
or 4 weekend days in a 4-week schedule. The clinician must be able
to float within the scope of the case management services to cover
gaps as needed. Candidates can expect to work Monday to Friday
during business hours. The ability to coordinate care and conduct
needs assessments is crucial. Familiarity with Medicare and
Medicaid Services is essential. Knowledge of HIPAA guidelines is
expected. Interdisciplinary support will be available. Experience
with Epic charting is required but may be negotiable for the right
candidate. The role involves working with various age groups,
including neonates, adolescents, adults, and geriatrics. Weekend
coverage is necessary to meet departmental needs. A commitment to
patient care and support is vital in this role. References are
required from supervisory roles in a comparable specialty within
