Job Title: Social Worker Profession: Social Worker Specialty: Wellness Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: Minimum years of experience in long-term care and funding procurement for individuals needing long-term care required. License: State Social Work license Certifications: Basic Life Support (BLS) from the American Heart Association Must-Have: - Long-term care experience - Expertise in funding procurement for individuals in long-term care Description: Assures continuity of care during the transition from hospital to post-hospital care. Assists patients and their families with personal and environmental difficulties as they transition to home or other healthcare providers. Serves as a primary advocate for residents, working as part of the interdisciplinary care team. Supports residents in maintaining their highest level of activity and life enrichment in meaningful ways. Oversees resident funding procurement before admission. Collaborates with the financial coordination team and resident representatives to ensure ongoing funding throughout their stay. Aligns responsibilities with federal and state regulations. Performs other duties within the scope of practice to support high-quality resident care. Utilizes EPIC charting, Outlook, auditing software, etc. Monday through Friday schedule from 8:00 AM to 4:30 PM. Business casual dress code. Department-specific orientation and general traveler orientation provided.
