MEDICAL PROVIDER JOB SUMMARY

Responsible for the overall care of all patients of assigned clinic. Performs physical exams, physical assessments, as well as prescribing and managing the methadone treatment plan of each patient. Works directly with Clinic Manager and clinical staff as needed for medical and treatment needs of clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Includes the following. Other duties may be assigned)

* Documentation

Responsible for ensuring that all medical evaluations, medical histories, and physical examinations have been completed before patient receives initial dose. Also responsible for ongoing treatment of patients including signing medical orders, treatment plans, and documentation required by state and federal laws
* Case Management

Responsible for patient's case management including chronic health problems, acute self limited problems, health conditions, and the management of medication prescribed by Community Medical Services.
* Referrals

Evaluation and referral of psychiatric condition and case management of patients under psychiatric care. Medical evaluations as needed to ensure that all healthcare needs are being met.

QUALIFICATIONS

* Licensed as a Physician in the state where they are employed by Community Medical Services
* Knowledge of substance abuse and the disease process of addiction
