Job Description:

The Patient Care Coordinator I is the point of contact to ensure patients' needs are met. The PCC I coordinates patients' treatment plans and follow up on their needs. Essential Functions · Determines insurance eligibility and coverage for a variety of plans and patient-specific situations · Submits authorization requests to insurance in an accurate and timely manner · Communicates with patients and families to assess supply needs · Screens/rescreens patients for nutrition risk per established criteria · Documents data collection results and makes appropriate referrals to the Dietitian as per · Communicates effectively with physicians and dietitians regarding patient needs and required documentation · Works directly with staff, patients, insurance companies and referral sources to ensure the best possible outcomes for our patients · Documents communication and action plans thoroughly · Troubleshoots product sourcing issues if applicable · Special Projects as assigned · Process customer orders, requests and information updates into a patient order management system · Verify insurance coverage for all insurance carriers. · Work with vendors staff to confirm order accuracy and details, and occasionally ship orders as needed Required skills 1. Experience recognizing the needs and concerns of individuals 2. Effectively communicate orally and in writing 3. Ability to follow verbal and written instruction 4. Professional telephone & email etiquette 5. Able to work in fast-paced, self-directed environment. 6. Detail oriented with excellent time management skills. 7. Decision-making, problem resolution and creative thinking skills. 8. Responds well to multitasking while providing exceptional customer service, support and responsiveness 9. Should be honest, competitive, energetic, enthusiastic, professional and systematic. 10. Ability to demonstrate and educate to patient family members on all equipment necessary Competencies 1. Shows proficiency in insurance verification 2. Documents communication and action plans thoroughly 3. Processes customer orders, requests and information updates into a patient order management system 4. Adequately Screens/rescreens patients for nutrition risk per established criteria 5. Submits authorization requests to insurance in an accurate and timely manner 6. Demonstrates accuracy and thoroughness 7. Observes safety and security procedures 8. Responds promptly to customer and patient needs 9. Shows proficiency in inventory management and optimization 10. Adequately navigates patient portal and follows set procedures Supervisory Responsibility None Work Environment Remote Physical Demands Able to lift and carry up to 10lbs Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m. and On-Call Travel None Preferred Education and Experience · Associate degree or Equivalent experience
