Patient Care Coordinator (Outpatient) at Providence Health and
Services summary: The Patient Care Coordinator (Outpatient) manages
and coordinates patient services within a multidisciplinary care
team, focusing on assisting patients in navigating the healthcare
system and managing specialty referrals. The role involves
collaborating with external service providers to ensure seamless
transitions of care and providing patient education on self-care
and community resources. Required qualifications include relevant
healthcare education or experience in an outpatient setting,

environment. Description The Clinical Care Coordinator participates
as an integral member of the multidisciplinary care team at PMGA.
Key roles are coordination of services for panel patients, focusing
on those who face particular difficulty with navigating the
healthcare system, management of specialty referrals and supports
patients in self-care and identifying relevant community resources.
The Clinical Care Coordinator collaborates with and outreaches to
the outside service providers to provide seamless transitions of
care, and provides patient education while assisting with access to

Qualifications: Associate's Degree in Healthcare Related, or in
lieu of Associate's degree: 2 years of experience in an outpatient
setting OR related coursework/training and 1 year of experience in
an outpatient setting Preferred Qualifications: Bachelor’s Degree
in Healthcare Related 2 years care coordination experience.

position. The successful candidate's placement on the wage range
for this position will be determined based upon relevant job

pay range; additional compensation may be available for this role,
