Job Description:
Clinical Referral & Care Coordinator at Providence Health and
Services summary: The Clinical Referral & Care Coordinator manages
specialty referrals and coordinates services for patients facing
challenges navigating the healthcare system. They collaborate with
multidisciplinary teams and external providers to ensure seamless
care transitions and provide patient education and resource
support. The role requires healthcare-related education or
experience, preferably in outpatient care, and focuses on improving
patient outcomes through effective care coordination. Description
The Clinical Care Coordinator participates as an integral member of
the multidisciplinary care team at PMGA. Key roles are coordination
of services for panel patients, focusing on those who face
particular difficulty with navigating the healthcare system,
management of specialty referrals and supports patients in
self-care and identifying relevant community resources. The
Clinical Care Coordinator collaborates with and outreaches to the
outside service providers to provide seamless transitions of care,
and provides patient education while assisting with access to

Qualifications: Associate's Degree in Healthcare Related, or in
lieu of Associate's degree: 2 years of experience in an outpatient
setting OR related coursework/training and 1 year of experience in
an outpatient setting Preferred Qualifications: Bachelor’s Degree
in Healthcare Related 2 years care coordination experience.

position. The successful candidate's placement on the wage range
for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
