Job Description:
Akdialysis is seeking a dedicated and compassionate Home
Therapies Nurse to join our team, focused on transforming the lives
of patients with chronic kidney diseases through pioneering and
personalized care. This role involves administering dialysis
treatment to patients in the comfort of their own homes, educating
patients and their families on their health condition, and
continuously monitoring patient health. The ideal candidate will be
deeply committed to providing exceptional patient care, adhering to
the highest standards of safety and hygiene. Administer home
peritoneal dialysis treatments to assigned patients in compliance
with prescribed physicians orders and policies of Akdialysis.
Assess and monitor patients’ treatment care plans, including vital
signs and lab work, and coordinate with other healthcare team
members to optimize care plans. Educate patients and families on
dialysis treatment, management of side effects, and home safety
procedures. Document all patient care activities and treatment data
accurately in the patient’s medical record. Ensure operation of
dialysis equipment by following manufacturer's instructions and
performing preventive maintenance. Manage supplies and equipment
inventory to ensure all necessary materials are available for each
patient visit. Travel to patients’ homes within the designated
service area, applying excellent time management skills to maintain
the punctuality and frequency of visits. Proficient in
administering peritoneal dialysis and understanding of
dialysis-related equipment. Strong interpersonal and communication
skills for effective patient and family education. Ability to work
independently with minimal supervision. Exceptional organizational
and time management skills. Medical Insurance Dental Insurance
Vision Insurance 401k Valid Registered Nurse (RN) license in the
state of practice. CPR certification. CNN (Certified Nephrology
Nurse) or CDN (Certified Dialysis Nurse) certification
preferred.
