Social Work is a consult based service. Clinicians complete psychosocial assessments and identify appropriate discharge plans from a person centered approach and in alignment with policy and regulations. Social Work Clinicians provide brief clinical interventions as appropriate, assisting with resource navigation and coordination of care, screen for suicidality and engage in safety planning as appropriate, complete appropriate reports of harm based on state statutes, and coordinate with outside entities. SW is split into three areas, Emergency Department, Children's, and Adult teams. The Children's Hospital team supports pediatrics, PICU, NICU and maternity units while the Adult team focuses on all inpatient units with patients over the age of 18. Clinicians work closely with RN Case Managers and the care team to coordinate and carry out safe and ethical discharge plans. Special Equipment : Degree can be any master's social work degree as long as it's from a CWSE accredited school of social work. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility. Facility only accepts Hep B vaccines and will not take titers or declination REFERENCES - Minimum of 2 supervisory references covering 1 year (6 months for CNA s) of employment total within the last three years within a comparable specialty/setting that they are booked. This can include Charge RNs / Leads. If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
