Trinity Staffing Group is seeking a travel nurse RN Skilled Nursing Facility (SNF) Skilled Nursing for a travel nursing job in Anchorage, Alaska. Job Description & Requirements - Specialty: Skilled Nursing - Discipline: RN - Start Date: ASAP - Duration: 13 weeks - 36 hours per week - Shift: 12 hours, nights - Employment Type: Travel Post Acute Facility • Perform initial assessments on admission — evaluate physical, mental, skin integrity, mobility, nutrition status, etc. • Monitor vital signs, fluid balance, weight, input/output. • Observe for changes in condition (e.g. signs of infection, pain, adverse reactions) and report to physician or advanced practice provider. • Monitor wounds, risk of falls, skin breakdown. • Work with interdisciplinary team (physicians, therapists, dietitians, social services) to create individualized care plans. • Update care plans as patient's needs change. • Ensure therapies (PT/OT etc.) are integrated into plan. • Discharge planning: coordinate with patient/family to transition to home or next level of care.
