About Twin Cities Physicians: Twin Cities Physicians provides mobile primary care services to residents in over 200 senior living communities across Minnesota. Position Summary: We are seeking a motivated Nurse Practitioner (AGNP or CNP) to deliver on-site care to senior living communities. If you’re passionate about serving older adults and want to expand into wound care, this is your chance. We’ll even pay for your wound care certification if you’re not already credentialed. Key Responsibilities: Deliver compassionate primary care in senior living communities Develop and manage care plans, provide wound care, and support chronic disease management Coordinate with interdisciplinary teams, families, and support staff Keep documentation current and clear in EMR systems Qualifications: Current CNP license in Minnesota AGNP certification preferred Geriatric or primary care experience preferred Reliable transportation and valid auto insurance Strong communication and clinical judgment Military Occupational Classification (MOC) Codes: Army: 66P – Nurse Practitioner Navy: 1972 – Nurse Practitioner Air Force: 46Y3H – Nurse Practitioner Marines: NP – Nurse Practitioner Coast Guard: HS – Health Services Technician Join Our Team: Ready to elevate your NP career while making a real difference in seniors’ lives?
