Job Description:
The Southcentral Foundation (SCF) Community Health Aide is
responsible for providing acute, chronic, emergency, and preventive
medical health care to Village residents under the direction of the
medical doctors in Anchorage and according to the CHA/P Manual. The
CHA I, II, III, IV and Practitioner are expected to operate within
the guidelines of the CHA/P Manual and their level of training and
the scope of practice consistent with their level of training. CHA
II, III, IV and Practitioner are also expected to act within the
limits of their Standing Orders (SO) when applicable.
Minimum Qualifications:
- High School diploma; OR GED.
Additional Qualifications for CHA I:Meets all requirements of CHA
Trainee in addition to the following:
- Successful completion of the TABE Test at the 9th Grade
level.
- Successful completion of Emergency Trauma Technician (ETT) OR
Emergency Medical Technician. Certification must be maintained
throughout employment.
- Complete full-time work in a Village clinic with 20
customer-owner encounters prior to Session II of the CHA Training.
Involves direct onsite observation, Medical Traffic Evaluation
[three (3) calls], PEF chart review [five (5) charts or notes] and
completion of the PSLN (Post Session Learning Needs) form.
- Successful completion of the Session I [one (1)] of CHA
training program and passing required testing with a grade of 80
percent or better.
- All certifications must be maintained at current level.
Additional Qualifications for CHA II:Meets all requirements of CHA
I in addition to the following:
- Successful completion of the Session II [two (2)] of CHA
training program and passing required testing with a grade of 80
percent or better.
- Complete 200 hours of field experience including 60
customer-owner encounters before the next training session.
Requires direct onsite observation, Medical Traffic Evaluation
[three (3) calls], PEF chart review [five (5) charts or notes] and
completion of the PSLN (Post Session Learning Needs) form.
Additional Qualifications for CHA III:Meets all requirements of CHA
II in addition to the following:
- Successful completion of the Session III [three (3)] of CHA
training program and passing required testing with a grade of 80
percent or better. Completion of 200 hours of field experience,
including 60 customer-owners encounters prior to the next session
of training.
- This will also involve direct onsite observation, Medical
Traffic Evaluation [three (3) calls], PEF chart review [five (5)
charts and notes] and completion of the PSLN (Post Session Learning
Needs) form.
- Will apply for certification after completing required field
training for this level.
- Current certification by the CHA Program Certification Board
(CHAPCB).
- All certifications must be maintained at current
level.
- In accordance with the Patient Protection and Affordable Care
Act and the National Correct Coding Initiative (NCCI) it is
required to enroll as individual rendering providers with Alaska
Medicaid.
Additional Qualifications for CHA IV:Meets all requirements of CHA
III in addition to the following:
- Successful completion of the Session IV [four (4)] of CHA
training program and passing required testing with a grade of 80
percent or better.
- Completion of an additional 200 hours of field experience,
including 60 customer-owner encounters or alternative completed to
the satisfaction of the designated Supervisor. This will also
involve direct onsite observation, Medical Traffic Evaluation
[three (3) calls], PEF chart review [five (5) charts and notes] and
completion of the PSLN (Post Session Learning Needs) form.
- Current certification by the CHA Program Certification Board
(CHAPCB), Level IV.
- All certifications must be maintained at current
level.
- In accordance with the Patient Protection and Affordable Care
Act and the National Correct Coding Initiative (NCCI) it is
required to enroll as individual rendering providers with Alaska
Medicaid.
Additional Qualifications for CHA Practitioner:Meets all
requirements of CHA IV in addition to the following:
- Successful passing of a preceptorship through one-on-one
evaluation by CHA Preceptor for 30 hours minimum direct
customer-owner care, performing at least 15 customer-owner
encounters as primary provider.
- Complete credentialing as a CHP with the training center per
guidelines.
- After credentialing as a CHP, complete 48 hours of approved
Continuing Education every two (2) years.
- The preceptorship and re-credentialing is to be repeated every
six (6) years.
- Current certification as a Practitioner by the CHA Program
Certification Board (CHAPCB).
- All certifications must be maintained at current
level.
- In accordance with the Patient Protection and Affordable Care
Act and the National Correct Coding Initiative (NCCI) it is
required to enroll as individual rendering providers with Alaska
Medicaid.
Employee Health Requirements:
Compliance with our Employee Health procedure is a condition of SCF
employment.You are required to agree that you will comply with all
job-related employee health screening and immunizations prior to
your first day of employment. Jobs designated as a Health Care
Personnel (HCP) position, requires that you have documentation that
you have completed the following immunizations prior to your first
day of employment: MMR (Measles, Mumps and Rubella, Varicella
(Chicken Pox), Hepatitis B, Influenza, and T-dap (Tetanus -
Diphtheria - Pertussis) COVID-19 Vaccine.
