Job Description:
Description Providence is calling a Licensed Practical Nurse
Primary Care (LTC Nursing, Variable Shift, 0.9 FTE) to Seward
Medical Center in Seward, Alaska. We are a community of caregivers

vulnerable. $7,500 Hiring Bonus for eligible external candidates

that begin on your first day of employment. In this position you
will:
- COMMUNICATE: Closely with the elder and their family. Keeps the
elder and family informed of needs or changes.
- SUPERVISE:
- Overall care for a specific group of elders within scope of
practice according to licensure.
- This includes performing chart reviews, to include but not
limited to, eMARs, eTARs, and provider orders match; ensure that
elder needs are addressed, documentation is accurate, and
evaluating effectiveness of interventions; and review elder care
plan for accuracy.
- This includes review of progress notes for any plans that may
not have been brought forward at the time of the writing of the
periodic assessment note to the care plan.
- ADMINISTER: Medications and treatments, and provide direct
licensed nursing care for a specific group of elders on a given
shift.
- CONTACT:
- Other members of the clinical support team, as needed.
- Complete provider orders, ensuring diagnosis, or condition with
each order. Evaluate justification for each medication as medically
necessary.
- Coordinate efforts to reduce medications.
- Refer to nursing or social worker intervention prior to gaining
an order for a psychoactive medication.
- Consuls RN Team Advisor for possible plan of care changes prior
to requesting medication or calling the provider.
- Alert RN Team Advisors, Mgr. LTC Nursing, DNS, and Employee
Education Nurse of needed education.
- REPORT: To the Shahbazim and gains a return report from the
Shahbazim during the shift.
- COLLABORATE: With and report significant events, findings, and
change of condition to RN Team Advisors and clinical support team
for service needs of elders.
- ASSESS: Elders within the scope of practice according to
licensure.
- NOTIFIES:
- The providers of elder changes of condition and receives
orders.
- Upon a change of condition, will maintain close communication
with the RN Team Advisors for further assessment.
- RN Team Advisors will determine if the change of condition
needs further changes to MDS and care plans.
- ATTENDS AND PARTICIPATES: In elder health care
conferences.
- ACTS AS A CLINICAL RESOURCE
- Role models collaboration and partnership with providers,
caregivers, elders, family and the rest of the team.
- Participates in the orientation and training of new staff
caregivers.
- COMPLIANCE:
- Direct and administer assigned functions to ensure high
quality, cost effective elder services which meet or surpass survey
standards.
- Assure adequate preparation and participation in regulatory
compliance surveys
- Demonstrate awareness of human and elder rights in all aspects
of care, treating each elder and co-worker with respect and
dignity
- Seek to expand personal professional growth by individual study
of current information related to current nursing techniques and
Long Term Care specialties.
- Attend meetings and in-services including mandatory and
interdepartmental in-services as assigned.
- Complete Competency Plan for assigned job and department.
Required qualifications for this position include:
- Graduate of an accredited school as a Licensed Practical
Nurse
- Current Alaska Registered Nurse (RN) license
- Basic Life Support (BLS) certification at time of hire from one
of the following governing body
- American Heart Association (AHA) - (BLS Provider)
- American Red Cross (ARC) - (BLS for Health Care Provider)
- Military Training Network (MTN) - (Healthcare Provider)
Preferred qualifications for this position include:
- 6 months nursing experience in a health care facility
