This is a part-time (20 hours a week) opportunity that will be
traveling in Anchorage, Alaska to see patients.
Social Worker MSW at Providence In Home Services
Provides professional social work services to improve or maintain
the social, emotional, functional and physical health status of the
client and support systems. Works in collaboration with client,
family, physician, other staff and community agencies to assess,
plan, implement and evaluate appropriate treatment. Collaborates
with interdisciplinary team in the process of assessment,
evaluation and coordination of services for client and/or family.
Assists to identify, interpret and reduce psychosocial factors
contributing to illness. Interviews, coordinates and plans services
to meet the social and emotional needs of clients and/or family.
Provides case management and assists with referral process.
Provides social work consultation to agency.
- Master's Degree in Social Work
- Alaska Masters Social Worker License upon hire
- 2 years of experience in acute care medical social work
- Must have experience with patient and family assessment and
diverse populations to include the chronically ill, older adult and

Providence Home and Community Care (HCC) is a service line within
Providence with over 8,000 caregivers and 2,000 volunteers serving
