Description: Providence is calling a Social Worker (MSW)
(Hospice, Day Shift, 0.0 FTE/On Call) to Providence Hospice in
Anchorage, Alaska. This position would be working Monday-Friday (no
weekends). We are a community of caregivers delivering every day on

you will: Provide individual, family, and group therapy to
patients, families and significant others. Complete a comprehensive
assessment of the psychosocial needs of the patient/family based on
admission criteria and the identified needs and problems of the
patient/family within 5 days of initial patient contact.
Participate in the development of the initial and ongoing
interdisciplinary plan of care. Consult with interdisciplinary team
members, departments, and health care facilities regarding the
ongoing coordination of patient care, documenting updates and
changes on the interdisciplinary Plan of Care. Actively
participates in IDG meetings and follows up with issues related to
the Plan of Care promptly, reporting back to the group as
appropriate. Complete clinical and progress notes within 24 hours
of patient contact, assuring they are part of the permanent medical
record in accordance with the established policy of the Hospice and
Palliative Care Programs. Provide counseling and support to
patients and families as they adjust to a terminal illness, altered
body image, changing roles within the family, family dynamics and
conflicts. Assists with funeral planning, financial concerns and
the access and use of community resources. Provides bereavement
support in collaboration with the IDG and Hospice Bereavement
Program. Facilitate access and utilization of appropriate community
resources beyond the scope of Hospice or Palliative care, such as
long-term skilled care, psychiatric care, and drug and alcohol
treatment. Provide education on a limited basis to the Hospice and
other health care program staff, community agencies, and physicians
to enhance their understanding of services and the psychosocial
factors in the care of the patient. Develop professional expertise.
Maintains ongoing knowledge of current medical social work theories
and treatment modalities. Maintains current licensure status by
meeting CEU requirements. Work pro-actively with the Hospice
Volunteer Program as directed by the Hospice Manager. In
coordination with the Hospice Manager and PAMC Volunteer
Coordinator orients, trains, and coordinates the activities of the
Hospice volunteers and Volunteer Program. Assures volunteers comply
with established policies and procedures and reports issues
promptly to the Hospice Manager for resolution. Support department,
region, and System Quality Improvement Plans in compliance with
State, Federal and other regulatory guidelines. Qualifications:
Required qualifications for this position include: Master's Degree
in Social Work. Alaska LMSW or LCSW. Must maintain current
licensure for appropriate level of education. Valid Drivers License
and automobile insurance according to AK State Motor Vehicle laws
are required. One (1) year medical or bereavement social work
experience Preferred qualifications for this position include: Two
years of supervised, post-masters experience in a psychiatric or
