As a RN Care Manager, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients and their families.Key Responsibilities

• Assess patient needs and develop individualized care plans
• Coordinate with healthcare teams to ensure seamless care delivery
• Provide education and support to patients and families
• Monitor patient progress and adjust care plans as neededRequirements
  • Graduate of an accredited school of nursing
  • Alaska Registered Nurse License upon hire
