Description Care Manager RN - AK PMG Peds Neuro at Providence Physician Service Organization The RN Case Manager will work as a care team member to support the needs of the clinics and assume responsibility for the ongoing case management of an assigned panel of patients. As a case manager, the RN will coordinate with a multidisciplinary team that includes patients, families, physicians, medical assistants, social workers, counselors, and other clinic support staff to provide high quality care. Required qualifications:

• Graduation from an accredited school of nursing.
• 2 years Professional nursing experience.
• Required upon hire Alaska Registered Nurse License (Vendor Managed) Preferred qualifications:
  • Bachelor's Degree in Nursing.
  • 2 years Case Management in a clinic or outpatient setting.
