About the Position Job Title: Case Manager RN Profession:

Shift: Day Hours per Shift: 5x8 (07:30 - 16:00) Experience: Minimum
1 year License: RN license required Certifications: BLS, ACM
Certification preferred Description: The candidate must have an RN
license in hand for auto offer. The work schedule consists of 5x8
day shifts. Shift times may vary, typically one of the following:
0700-1530, 0730-1600, 0800-1630, or 0830-1700. First-time travelers
are accepted. Experience in acute care is required. Epic experience
is required. Candidates must be available to work every other
weekend or four weekend days in a four-week schedule. Candidates
must float within the scope of case management services as needed.
Individuals must be prepared to work with pediatric, adult, and
geriatric populations. The facility supports an interdisciplinary
approach including various medical and therapeutic teams.
Responsibilities include discharge planning, needs assessment, care
coordination, and planning of care. Candidates are expected to
follow HIPAA guidelines and CMS regulations. Preferred skills
include familiarity with Milliman Guidelines. The role requires
flexibility to accommodate department needs. Candidates must not
have a permanent address within 50 miles of the facility.
References are required from supervisory roles within the last
three years. Travelers can work a maximum of one calendar year in
the same assignment. A break of at least six months is required
before returning to a new travel assignment. Requirements Required
